29 CFR 1910.179(n)(3)(i):The load was not well secured and properly balanced in the sling or lifting device before it was lifted more than a few inches: Employees were exposed to struck-by hazards. Employees engaged in material handling operations utilizing the overhead crane on Laminator #2, were exposed to struck-by hazards in that the load was not well secured before being lifted.

29 CFR 1910.303(g)(2)(i):Exposed live electrical parts were not protected from accidental contact by an approved enclosure. Employees were exposed to shock hazards. Employees engaged activities near the opened chiller control panel located in the boiler room were exposed to shock hazards in that the electrical control panel located on chiller #1 did not have a guarded enclosure.
