On or about 10/30/2021, on the concrete pad, the Caterpillar model 924H front end loader, serial number C6E22487J2Y03029, was operated with damaged equipment, such as but not limited to shattered window and inoperable lights, horn, and windshield wipers to move charcoal from the kilns to the pad.
