29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices. (a) In the production area, on or about January 29, 2026 and at times prior to, the employer exposed employees to chemical exposure hazards in that documentation for a pressure relief device (PRD) still in use after being activated on November 21, 2026 was not maintained or available for the PRD to show that it complied with recognized and generally accepted good engineering practices (manufacturer guidelines).

29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information on paragraph (d) of 29 CFR 1910.119) before further use or in a safe and timely manner when necessary means were taken to assure safe operation. (a) In the production area, on or about January 29, 2026, the employer exposed employees to chemical exposure hazards in that, deficiencies in a pressure relief valve (PRV) were not corrected before being reinstalled.
