OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  The employer is failing to protect employees from struck-by hazards. This violation was most recently documented on November 1, 2021 at a worksite located at or near 2701 W Omaha, Norfolk, Nebraska.  An employee inspecting a pipe section in the travel path of a Yale automated hoist, exposed the employee to being struck by the hoist as demonstrated in this incident in which an employee was struck by the hoist while in a scissor lift.  Among other method, feasible and acceptable means of abatement are:  Develop and implement safe work practices and procedures to eliminate struck-by hazards when employees work in the path of travel of automated overhead hoists.. The safe work practice procedures shall utilize a risk assessment, a hazard identification, a task identification and hazard elimination and risk reduction as outlined in ANSI RIA R 15.06 -2012 paragraphs 4.3.1/4.4.1/4.4.2/4.5.   Employees shall be trained over safe work practice procedures that include, but not limited to the following:   (1) Establishing a safe zone where employees can perform work tasks.  (2) Establishing a procedure to ensure that automated hoists are locked out prior to any work performed the path of the hoist.  (3) Establishing a training program that includes hazard identification and elimination of work performed in the path of automated hoists.
