Cumberland Knob, Milepost 217.5: On or before October 26, 2021, the employer did not conduct inspections to ensure the dilapitated fuel storage shed was properly repaired. Employees were exposed to struck-by hazards.

Cumberland Knob, Milepost 217.5: On or about October 26, 2021, multiple electrical panels were missing blank knockout covers. Employees were employees to live parts.
