29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a) At 210 A Angelfish Ave., Fort Walton Beach, Florida: On or about October 27, 2021 and at times prior; the employer allowed his employees to operate a circular saw without eye protection while cutting wood planks exposing his employees to struck-by hazards.

29 CFR  1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  a) At 112 Amberjack Dr., Fort Walton Beach, Florida: On or about October 7, 2021 and at times prior; the employer allowed his employees to perform framing activities where there was an opening in the floor approximately 9  feet that was not protected with any type of guardrail system, exposing employees to a fall hazard of 30 feet to the ground below.
