29 CFR 1910.212(a)(3)(iii): Special hand tools for placing and removing material(s) was not provided to permit easy handling of material without the operator placing a hand in the danger zone: On or about October 18, 2021 and at times prior thereto, the employer did not ensure special hand tools were used when the machine guarding was not feasible to prevent employees from placing their hands into CNC bender #3 point of operation, exposing employees to amputation hazards. Employees placed and retrieved tubing with their hands in the point of operation, exposing the employees to amputation hazards. The ATCO Products, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(3)(iii), which was contained in OSHA inspection number 1536748, citation number 1, item number 1 and was affirmed as a final order on September 21, 2021, with respect to a final abatement date on September 10, 2021, with respect to a workplace located at 601 I-45 SOUTH FERRIS, TX 75125.

29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. On or about October 18, 2021, the employer did not report to OSHA a work related amputation within 24 hours of being notified of the employee injury in the CNC Bending Department located in Ferris, Texas. The ATCO Products, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1904.39(a)(2), which was contained in OSHA inspection number 1536748, citation number 2, item number 1 and was affirmed as a final order on September 21, 2021, with respect to a final abatement date on June 17, 2021, with respect to a workplace located at 601 I-45 SOUTH FERRIS, TX 75125.
