29 CFR 1910.1200(e)(1):Employer had not developed or implemented a written hazard communication program included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii): a) Covert Manufacturing, Inc. located at 328 South East Street in Galion, Ohio 44833: On or about November 4, 2021, the employer did not implement a written hazards communication program which describes how criteria specific for labeling chemical hazards in the work place will be met. Portable containers of Purple Cleaner and Degreaser Concentrate and Spray HIB 6000-B were not labeled with the product identifier and words, pictures, symbols, or a combination thereof, which provide at least general information regarding the hazards of the chemical contents. Employees operating CNC machines were exposed to hazards associated with sodium hydroxide, butoxyethanol, and sulfonic acids, such as, but not limited to burns to the eyes and skin, and irritation to the respiratory tract.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical: Covert Manufacturing, Inc. located at 328 South East Street in Galion, Ohio 44833: a) On or about November 4, 2021, the employer did not ensure portable containers of Purple Cleaner and Degreaser Concentrate were labeled with the product identifier and words, pictures, symbols, or a combination thereof, which provide at least general information regarding the hazards of the chemical contents. Employees operating CNC machines were exposed to hazards associated with sodium hydroxide, butoxyethanol, and sulfonic acids, such as, but not limited to burns to the eyes and skin, and irritation to the respiratory tract. b) On or about November 4, 2021, the employer did not ensure a portable container of SprayHIB6000-B was labeled with the product identifier and words, pictures, symbols, or a combination thereof, which provide at least general information regarding the hazards of the chemical contents. Employees in the coolant department were exposed to hazards associated with Chemtrol Spray HIB 6000B such as, but not limited to, eye, skin and respiratory irritation, and aggravation of preexisting medical conditions.

Employees handling Purple Cleaner and Degreaser Concentrate and Spray HIB 6000-B were exposed to hazards associated with sodium hydroxide, butoxyethanol, and sulfonic acids, such as, but not limited to burns to the eyes and skin, and irritation to the respiratory tract.
