Large cracks and missing chunks of concrete were not covered, sealed and/or repaired with other approved means provided where industrial trucks travel and pedestrians walk. 29 CFR 1910.178(m)(7): Brakes were not set and/or wheel blocks were not in place to prevent movement of trucks, trailers, or railroad cars while loading and unloading: a- American Linehaul Corporation,333 1st Street, Elizabeth, NJ.- Warehouse - At the loading dock, employees were exposed to a struck by and caught in hazard, in that, measures to prevent the displacement of trailers while loading and offloading were not taken at all bay doors. Condition noted on or about 11/5/21.
