29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition: (a) On or about November 5, 2021,, at the worksite located at 650 S. 108th St., West Allis, WI, the employer did not ensure the floor surfaces, where personnel traverse, both on foot and in powered industrial vehicles, was inspected and maintained free of tripping hazards. Multiple surfaces of the warehouse area had sections where the concrete was missing, thus, exposing employees to trip hazards.
