On or about 11/08/2021, Hector Pineda DBA Hector Pineda was installing roofing materials on a roof of a commercial building located at 1203 East Veterans Memorial Parkway in Warrenton, MO.  Three employees were installing roofing materials on a 3/12 sloped roof, approximately seventeen feet above the ground, without fall protection.  They were not using guardrail systems, safety net systems, or personal fall arrest systems or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.
