Teijin Automotive Technologies on or about November 08, 2021, at the address of 100 S. Poe Rd. N. Baltimore, OH: The employer did not ensure that employees followed procedures for verifying employees were safely positioned or removed from the work area before removing lockout lock on press 4 robotic cell. In this condition employees are exposed to injuries up to death.

29 CFR  1910.147(c)(7)(i):The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:  a.) Teijin Automotive Technologies on or about November 08, 2021, at the address of 100 S. Poe Rd. N. Baltimore, OH: The employer failed to ensure that employees who are exposed to energized rotating parts during service and jam clearing work on the #3 and #4 robotic cells were adequately trained in the skills required for the safe application, usage, methods, and means to isolate and control potential hazardous energy sources. The employees tasked with service work on the aforementioned machine were not specifically trained to lockout the machine to protect against amputation injuries prior to performing unjamming work near the point of rotation.

Teijin Automotive Technologies on or about November 08, 2021, at the address of 100 S. Poe Rd. N. Baltimore, OH: The employer did not ensure that the employee who applied the energy control lock and who was available at the facility removed their lockout lock on press 4 robotic cell prior to re-energization. Release from isolation without the initiating employee's verification exposed employees to struck-by and caught in hazards.

Teijin Automotive Technologies on or about November 08, 2021, at the address of 100 S. Poe Rd. N. Baltimore, OH: The employer did not require/ ensure that flexible cording located at press #6 used to power cooling fans was inspected prior to use. Employees were using damaged flexible cording, in this condition employees were exposed to injuries including but not limited to electrical shock.
