29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Teijin Automotive Technologies. on or about November 8, 2022, at the address of 2915 County Hwy 96 Carey, OH: The employer failed to ensure employees utilize energy control procedures on the Kenworth bonder machine prior to performing maintenance/servicing activities. Employees tasked with cleaning out pit that bonder carriage travels in, without the benefit of energy control, were exposed to crushing hazards.

29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Teijin Automotive Technologies, on or about November 15, 2022, at the address of 2915 County Hwy 96 Carey, OH: The employer did not ensure the Kenworth bonder machine was adequately guarded. Employees performing work task near the inadequately guarded bonder were exposed to crushing hazards created by the moving carriage and point of operation.
