29 CFR  1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  On or about November 9, 2021 an employee was not instructed to recognize and minimize unsafe conditions while operating a Skid steer loader, exposing employees to struck-by hazards.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR  1926.451(c)(2)(ii):Unstable objects were used to support scaffolds or platform units:  On or about November 9, 2021 a metal framed scaffold was erected on unstable objects, such as stacked boards, exposing employees to a possible scaffold collapse.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR  1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  On or about November 9, 2021 employees were not provided a safe means of access to the working section of the scaffold, exposing employees to a falls between 14-16 feet.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
