The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):   The employer is failing to protect employee(s) engaged in powered industrial truck operations from crushing and struck-by hazards. This was most recently documented on November 9th, 2021 in Omaha, Nebraska. The forklift operator had not completed forklift training to include a classroom portion, and written test that meets the requirements of OSHA standard 1910.178(l) powered industrial trucks, operator training.

29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  The employer is failing to protect employees from struck-by hazards. This was most recently documented on or about November 9th 2021, Omaha, Nebraska. An employee was engaged in residential framing activities using a skill saw. This employee was exposed to struck-by hazards because he was not utilizing eye protection.

This was most recently documented on or about November 9th, 2021 in Omaha , Nebraska. An employee was observed working on a platform that wasn't secured to the forklift - The approximate height the employee was working at was over 10 feet.

29 CFR  1926.451(g)(1):Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level.  The employer is failing to protect employees from fall hazards. This was most recently documented on or about November 9th, 2021, in Omaha, Nebraska. An Employee was engaged in framing activities and was exposed to fall hazards in excess of 10 feet when working from a  mobile scaffold. The exposed employee was working from a mobile platform with two unprotected sides.

This was most recently documented on or about November 9th, 2021 in Omaha, Nebraska. Upon entry to the residential structure, a hole was located on the main level of the residential structure. The hole was not protected by a guardrail or other fall protection system.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.   The employer is failing to protect employees from fall hazards. This was most recently documented on or about November 9th , 2021 in Omaha, Nebraska 68022.  Multiple  employees were observed working on a residential structure conducting framing operations. The exposed employees were standing on top of the top plates/trusses of the residential structure at heights over 6 feet- No fall protection was being utilized.
