29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: Location; site On or about 11/8/21, the employer did not ensure a guard rail system was installed on the side of the scaffolding in front of the wall openings. This exposed employees to a fall of approximately 12 feet.
