Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the struck-by hazards from approximately 100-300 lb. fabric roll loads should the lifting devices fail:  a)  Laser Cutting Area, on or about 11/10/2021: A 1/2 ton Coffing Model JLC Hoist was used for Laser No. 2 and Laser No. 4 to move approximately 100-300 lb. fabric rolls. The hoist was not inspected on a frequent and periodic basis and the throat latch was missing from the load hook.  b) Laser Cutting Area, on or about 11/10/2021: A 1/2 ton Coffing Model JLC Hoist was used for Laser No. 1 to move approximately 100-300 lb. fabric rolls. The hoist was not inspected on a frequent and periodic basis and the throat latch was missing from the load hook.  c) Laser Cutting Area, on or about 11/10/2021: A 1/2 ton Coffing Model Hoist was used for Laser No. 3  to move approximately 100-300 lb. fabric rolls. The hoist was not inspected on a frequent and periodic basis and the throat latch was missing from the load hook.

29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative  a) At the establishment, on or about 9/15/2021: Two Authorized Employees perform servicing and maintenance on machines including, but not limited to the Golden Laser Machines, Grommet Machines, and long and short arm Sewing Machines whose energy source are both electrical and pneumatic. Affected employees including, but not limited to Sewing Machine Operators, Laser Operators, and General Laborers work in and around the area where servicing or maintenance is being performed. The employer did not establish a written energy control program.

29 CFR  1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:   a)  At the establishment, on or about 10/13/2021: Employees were exposed to hazardous chemicals such as, but not limited to: red oil, polypropylene fabric, nylon fabric and polyester fabric during the manufacturing filter press cloth.  The employer did not provide information and training to employees on the hazardous chemicals in their work area as specified in 29 CFR 1910.1200(h)(1) and (2).
