29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: a. On or about November 3, 2021, at building 1 de-burr and polishing station, an orange extension cord, two relocatable power taps, and a movable 4-way plug were used as a substitute for fixed wiring. This exposed the employees to electrical hazards. b. On or about February 16, 2022, at building 1 de-burr and polishing, a yellow extension cord that powered an abrasive blasting cabinet was used as a substitute for fixed wiring. This exposed the employees to electrical hazards. c. On or about February 16, 2022, at building 1 de-burr and polishing, a yellow extension cord that powered a pump was used as a substitute for fixed wiring. This exposed the employees to electrical hazards. d. On or about February 16, 2022, at building 1 de-burr and polishing, a yellow extension cord that was plugged to an outlet and hung from overhead, was used as a substitute for fixed wiring. This exposed the employees to electrical hazards.

29 CFR 1910.94(a)(7): Operational procedures and general safety. Dust was permitted to accumulate on the floor and/or on ledges outside of an abrasive-blasting enclosure and/or dust spills were not cleaned up promptly and/or aisles and walkways were not kept clear of steel shot or similar abrasive as required. On or about October 26, 2021, building 1 de-burr and polishing area, the employer permitted dust to accumulate on the floor and on ledges outside of an abrasive-blasting cabinet and was not cleaned up promptly. This exposed the employees to slip hazards.
