On or about November 12, 2021  at jobsite located in 369 E. South St., Elmhurst, IL. 60126, employees were exposed to fall hazards approximately 20 feet while engaged in residential siding activities. Fall protection was not being utilized while performing these activities.

On or about November 12, 2021,  at jobsite located in 369 E. South St. Elmhurst, IL. 60126, the employer failed to certify that employee had been trained to recognize and minimize fall hazards. Employee were exposed to fall hazards approximately 20 feet while performing siding activities on a residential structure without using fall protection.
