29 CFR 1926.451(g)(4)(i):Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. On or about November 10, 2021, during a brick restoration project at a jobsite located at 2592 West 14th St, Cleveland, OH 44113: the employer did not ensure that a complete guard rail system was installed on all of the open sides and ends of the tubular welded frame scaffolding to protect employees from fall hazards, in that mid rails were missing to complete the guard rail system and scaffolding was used by employees at approximate heights greater than 60 feet.

29 CFR 1926.451(c)(2)(ii):Unstable objects shall not be used to support scaffolds or platform units. On or about November 10, 2021, during a brick restoration project at a jobsite located at 2592 West 14th St, Cleveland, OH 44113: the employer did not ensure that the scaffold footing was on a stable object, in that the stacked wood cribbing under the base of the scaffold footing was not secured together to prevent accidental displacement.
