Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews.  On or about November 10, 2021, during a brick restoration project at a jobsite located at 2592 West 14th St, Cleveland, OH 44113: the employer did not ensure that a complete guard rail system was installed on all of the open sides and ends of the tubular welded frame scaffolding to protect employees from fall hazards, in that mid rails were missing to complete the guard rail system and scaffolding was used by employees at approximate heights greater than 60 feet.
