29 CFR 1926.502(d)(15):Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows: On or about August 13, 2024, during the inspection, installation of scaffolding equipment, and repair of a roof top masonry chimney on a two-story residential home located at 3028 Prospect Avenue, Cleveland, Ohio, the employer did not ensure the anchorage used for attachment of the two employees personal fall arrest systems were secured to an anchorage capable of supporting at least 5,000 pounds per employee attached. The employees' personal fall arrest systems were anchored to a single 5/8" diameter lifeline safety rope, with a total of three employees attached to the same single lifeline safety rope.

29 CFR 1926.502(e)(2):Positioning devices shall be secured to an anchorage capable of supporting at least twice the potential impact load of an employee's fall or 3,000 pounds (13.3 kN), whichever is greater. On or about August 13, 2024, during the inspection, installation of scaffolding equipment, and repair of a roof top masonry chimney on a two-story residential home located at 3028 Prospect Avenue, Cleveland, Ohio, the employer did not ensure his positioning device system was secured to an anchorage capable of supporting at least twice that potential impact load of an employee's fall or 3,000 pounds. The employer's positioning device was anchored to a single 5/8" diameter lifeline safety rope, with a total of three employees attached to the same single lifeline safety rope.
