29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse: On or about November 18th, 2021, at the location of 30700 Aurora Road, Solon, OH, 44139: The employer did not ensure that product stacked on pallets in aisle "A" of the warehouse was stable and secured against sliding and falling. Employees were exposed to struck-by hazards as several stacks were leaning or had fallen against the storage racks.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: On or about November 18, 2021, in the warehouse located 30700 Aurora Road, Solon, Ohio 44319: Signage with a red arrow was present on the east wall, indicating the fire extinguisher location. The fire extinguisher was sitting on the floor instead of being mounted under the red arrow.
