29 CFR 1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a)  Meadowlark Log Homes Inc., Mezzanine South of Pad:  On or about November 18, 2021 and at times prior employees were exposed to a fall hazard of approximately 10 feet to the ground below, while utilizing the unguarded mezzanine to procure items such as, but not limited to; rolls of wrap, used in the manufacturing process.

29 CFR  1910.28(b)(11)(ii): The employer did not ensure that each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as described in 29 CFR 1910.28 Table D-2-Stairway Handrail Requirements.  a)  Meadowlark Log Homes Inc., Mezzanine Staircase:  On or about November 18, 2021 and at times prior employees were exposed to a fall hazard while accessing and egressing a staircase with 15 treads and 16 risers without a stair rail system.

29 CFR  1910.184(d):Each day before being used, the sling and all fastenings and attachments were not being inspected for damage or defects by a competent person designated by the employer:  a)  Ancra Sling (20-EE1-9802X3), Harrington Crane Serial 760479, Meadowlark Log Homes Inc:  On or about November 18, 2021 and at times prior employees were exposed to struck by hazards while utilizing damaged slings to maneuver log cabin components while manufacturing log homes.  The employer failed to ensure that all rigging equipment was inspected prior to each use.
