29 CFR 1910.95(c)(1):The employer shall administer a continuing, effective hearing conservation program, as described in paragraphs (c) through (o) of this section, whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level (TWA) of 85 decibels measured on the A scale (slow response) or, equivalently, a dose of fifty percent. For purposes of the hearing conservation program, employee noise exposures shall be computed in accordance with appendix A and Table G-16a, and without regard to any attenuation provided by the use of personal protective equipment. a) Meadowlark Log Homes: On or about May7th, 2025, and at times prior, employees were exposed to high levels of noise while working in and around the cabin manufacturing floor. Employee noise levels were measured at 100.7 and 98.4 decibels on an A-weighted scale (dBA) for a sampling period of 461 and 478 minutes, respectively, which is calculated to be 442.6% and 321.5%, respectively, of the action level dose (8-hour time-weighted average action level of 85dBA). A zero exposure was calculated for the remaining 19 and 2 minutes, respectively, for which sampling did not occur.

29 CFR 1910.101(b):The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: a) Meadowlark Log Homes, Mill shop: On or about May 7, 2025, and at times prior, employees were exposed to struck by hazards while working in and around unsecured cylinders such as, but not limited to: Argon gas cylinders.
