29 CFR  1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a) Alfredo Cazales Lopez DBA Cazales Roofing, LLC located at 1144 Walnut Street, Westerville, OH 43081:  On and preceding November 22, 2021, the employer did not ensure that a ladder used for access above the front porch of a single story residential home had the side rails extending at least 3 feet beyond the upper landing and the ladder was not secured at its top to a rigid support that will not deflect with a grasping device was provided.  The following condition exposed employees to falls up to 9 feet.

29 CFR  1926.20(b)(1):The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  a) Alfredo Cazales Lopez DBA Cazales Roofing, LLC located at 1144 Walnut Street, Westerville, OH 43081:  On and preceding November 22, 2021, the employer did not implement and maintain an effective safety and health program onsite to ensure that hazards were identified and corrected onsite.  Alfredo Cazales Lopez was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.20(b)(1), which was contained in OSHA inspection number 1348025, citation number 1, item number 1/b and was affirmed as a final order on October 24, 2018, with respect to a workplace located at 4014 Chelsea Corner, Hurricane, WV 25526 .

29 CFR  1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:   a) Alfredo Cazales Lopez DBA Cazales Roofing, LLC located at 1144 Walnut Street, Westerville, OH 43081:  On and preceding November 22, 2021, the employer did not ensure that eye protection was being utilized by roofers that were cutting wood with a circular saw and installing sheathing using a pneumatic nail gun which exposed the employees to struck-by hazards to the eye.  Alfredo Cazales Lopez was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1348025, citation number 1, item number 1/a and was affirmed as a final order on October 24, 2018, with respect to a workplace located at 4014 Chelsea Corner, Hurricane, WV 25526 .

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) Alfredo Cazales Lopez DBA Cazales Roofing, LLC located at 1144 Walnut Street, Westerville, OH 43081:  On and preceding November 22, 2021,  employees were performing re-roofing work without any form of fall protection, thereby exposed to a fall hazards ranging from 9-12 feet.  Alfredo Cazales Lopez was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1348025, citation number 1, item number 2/a and was affirmed as a final order on October 24, 2018, with respect to a workplace located at 4014 Chelsea Corner, Hurricane, WV 25526 .
