29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:   (a)	POOL ROOM: A 3-way splitter was used to provide power to a CAT controller, a white 120V Pre-Wired-back up Electrical Junction Box (Model HCSJBOX) connected to an Aquasol Controller (120V 1.7A), and a pump motor, exposing workers to electrocution and fire hazards.  (b)	POOL ROOM: Three 3-way splitters were used to provide power to three CAT controllers, and three white 120V Pre-Wired-back up Electrical Junction Box (Model HCSJBOX) each connected to an Aquasol Controller (120V 1.7A), exposing workers to electrocution and fire hazards.  (c)	MAIN KITCHEN SERVER STATION: A 15A relocatable power tap was connected to a Cornelius beverage machine, exposing workers to electrocution and fire hazards. (d)	ATRIUM BAR: a power strip was use to provide power for a blender and coffee pot, exposing workers to electrocution and fire hazard.

29 CFR  1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  (a)	3RD FLOOR LAUNDRY ROOM- A black extension cord (12G SJOOW 300V) was used to power the EcoLab chemical feed pump for the commercial washer and dryer, exposing workers to electrocution and fire hazards.   (b)	MAIN KITCHEN SERVER STATION: A Blue extension cord (16G SJTW 200V) was used to power a Bunn Tea pot (14.4A 120V SN TU00325505), exposing workers to electrocution and fire hazards.

The Exposure Control Plan did not include documentation of the last review date.
