29 CFR  1910.178(l): This is applicable to construction employment as referenced by 1926.602(d). The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):   The employer is failing to protect employee(s) engaged in powered industrial truck operations from crushing and struck-by hazards. This was most recently documented on November  19th, 2021 in Gretna, Nebraska. The forklift operator had not completed forklift training to include a classroom portion, and written test that meets the requirements of OSHA standard 1910.178(l) powered industrial trucks, operator training.

29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  The employer is failing to protect employees from struck-by hazards. This was most recently documented on or about November 19th 2021, Gretna, Nebraska. An employee was engaged in residential framing activities using a skill saw. This employee was exposed to struck-by hazards because he was not utilizing eye protection.

29 CFR  1926.1052(c)(1)(ii):Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stair rail system along each unprotected side or edge:  The employer is failing to protect employees from fall hazards. This was most recently documented on or about November 19, 2021 in Gretna, Nebraska. The stairwell was used by multiple employees for access and egress to the 2nd floor and it was not equipped with a stair rail system along each protected side or edge - the measurement from the lower level to the top landing area of the 2nd floor was over 9 feet.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  The employer is failing to protect employees from fall hazards associated with framing operations - This was most recently documented on or about November 19th, 2021 in Gretna, Nebraska -  Employees were observed working at heights greater than 6ft without using any form of fall protection.    HM Framers was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1335450, citation number 1, item number 1and was affirmed as a final order on 11-13-2018, with respect to a workplace located at 184th/Terrance, Omaha, Nebraska 68142.
