29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition.  a) Production Area: On or about November 23, 2021, and at times prior, the employer exposed employees to fire hazards in that combustible wood dust was allowed to accumulate resulting from the operation of equipment including, but not limited to, the Baker Band Resaw (Model BBR-O, S/N 97-2334) while employees were operating it.

29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   a) Production area: On or about November 23, 2021, and at times prior, the employer exposed employees to caught-in hazards in that they are required to change a band saw blade periodically on the Baker Band Resaw (Model BBR-O, S/N 97-2334) without following specific lock-out procedures. The employer has not established a program consisting of energy control procedures, employee training and periodic inspections.

29 CFR  1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:   a) Production Area: On or about November 23, 2021, and at times prior, the employer exposed employees to caught-in hazards, in that the conveyor's saw blade of the Morgan Bandsaw was not adequately guarded to prevent unexpected employee(s) contact during the operating cycle.  b) Production Area: On or about November 23, 2021, and at times prior, the employer exposed employees to caught-in hazards, in that the Notcher's point of operation was not adequately guarded to prevent unexpected employee(s) contact during the operating cycle.

29 CFR  1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:   a) Production Area, West Wall: On or about November 23, 2021, and at times prior, the employer exposed employees to machine hazards, in that access to the main electrical disconnects for equipment including, but not limited to, the Trim Saw Machine was obstructed by wood material which may delay or prevent isolation of electrical supply in an emergency involving the machine(s).  b) Production Area, East Wall: On or about November 23, 2021, and at times prior,  and at times prior, the employer exposed employees to machine hazards, in that access to the main electrical disconnects for equipment including, but not limited to, the Baker Band Resaw (Model BBR-O, S/N 97-2334) was obstructed by a wood dust exhaust pipe which may delay or prevent isolation  of electrical supply in an emergency involving the machine(s).
