29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: a. On or about November 18, 2021, at the warehouse southeast corner storage loft, employees were exposed to falls from a working/walking surface with an unprotected side or edge that was 8 feet (2.44 m) above a hard concrete lower level. b. On or about November 18, 2021, at the warehouse storage loft on top of the administration office, employees were exposed to falls from a working/walking surface with an unprotected side or edge that was 8 feet (2.44 m) above a hard concrete lower level. NOTE: Certification and documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c) and (d), respectively.

On or about November 18, 2021, at the warehouse, the employer did not ensure that the stairs connecting the ground level to the southeast corner storage loft, having 12 risers, were equipped with a stair rail system on each open side, as described in 29 CFR 1910.28 Table D-2 - Stairway Handrail Requirements. b. On or about November 18, 2021, at the warehouse, the employer did not ensure that the stairs connecting the ground level and the storage loft on top of the administration office, having 13 risers, were equipped with a stair rail system with handrail on the open side, as described in 29 CFR 1910.28 Table D-2 - Stairway Handrail Requirements. 29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment. a. On or about November 18, 2021, at the sheet metal shop; the electrical service panel in the area was not readily accessible. Access to the electrical service panel was obstructed by several sheet metal panels, of different shapes and sizes, stored in the floor below and in front. NOTE: Certification of abatement is not required for this citation item since it was submitted prior to citation issuance.
