(b) East Roof - Employees working from a residential roof were exposed to fall hazards of approximately twelve (12) feet without the use of guardrail systems, safety net system, or personal fall arrest system, on or about November 23, 2021. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   (a) South West Roof - Employees accessing a residential roof were exposed to a fall of approximately ten (10) feet in that the aluminum extension ladder used was not installed to allow the ladder side rails to extend at least three (3) feet above the upper lander surface, on or about November 23, 2021.  (b) East Roof - Employees accessing a residential roof were exposed to a fall of approximately twelve (12) feet in that the aluminum extension ladder used was not installed to allow the ladder side rails to extend at least three (3) feet above the upper lander surface, on or about November 23, 2021.  (c) Upper Roof - Employees accessing a residential roof were exposed to a fall of approximately twenty (20) feet in that the aluminum extension ladder used was not installed to allow the ladder side rails to extend at least three (3) feet above the upper lander surface, on or about November 23, 2021.
