The established procedure did not include steps for shutting down, isolating and securing the machine to control all hazardous energy on the Regloplas Hot Oil machine, including electrical, thermal, and chemical, procedural steps for the placement and removal of lockout/tagout devices, or specific requirements for testing the machine to determine and verify the effectiveness of the lockout devices and other energy control measures.
