29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) #16 Die Cast Line: On or about May 22, 2023, employees were exposed to the unexpected energization of the Buehler Evolution 600 Ton plunger during the change out of the shot rod tip as the equipment was turned off and not locked out. The employer failed to implement energy control application steps as the machines were not isolated from all energy sources [per the 1910.147(d)(3) requirements]. As a result, the remaining applicable energy control elements, involving LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) #16 Die Cast Line: On or about February 20, 2023, the employer did not ensure that hazards created by moving parts on equipment , such as the Buehler Evolution 600 Ton plunger, were guarded to prevent injury to employees while employees accessed the area during operation and observation.
