29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) 1116 Chadwick Circle, Hummelstown, PA- On or about November 22, 2021, employees were observed working on multiple levels of the roof removing shingles and felt without fall protection on an 8 pitch roof, exposing the employees to falls hazards between 10 to 20 feet. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) 1116 Chadwick Circle, Hummelstown PA, Back - On or about November 22, 2021, employees were observed accessing the roof via a ladder staged at the back of the house that was not extended 3 feet beyond the landing, exposing employees to a fall hazard. b) 1116 Chadwick Circle, Hummelstown PA, Side- On or about November 22, 2021, employees were observed accessing the lower roof from a ladder set up on the garage side of the house. The ladder was not extended 3 feet above the landing, exposing employees to a fall hazard. c) 1116 Chadwick Circle, Hummelstown PA, Front - On or about November 22, 2021, employees were observed accessing the roof via a ladder set up at the front of the house that was not extended 3 feet above the landing, exposing employees to a fall hazard. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. 29 CFR 1926.1053(b)(9): The area around the top and bottom of ladders was not kept clear: a) 1116 Chadwick Circle, Hummelstown PA, Side - On or about November 22, 2021, employees were accessing the roof using a Werner extension ladder on the side of the house, the base of the ladder was impeded by hedges, exposing employees to a fall hazard. b) 1116 Chadwick Circle, Hummelstown PA, Front- On or about November 22, 2021, employees were observed accessing the roof using an extension ladder at the front of the house that was partially covered with roofing debris, exposing employees to a contact hazard with sharp objects. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
