On or about November 30, 2021, a worker performed job tasks on top of the roof of an existing residential construction, and was not protected by guardrail system, safety net system, or personal fall arrest system. B. On or about November 30, 2021, workers performing job tasks near a basement opening of a residential construction rebuild were not protected from falling through the opening by personal fall arrest system, cover, or guardrail system. C. On or about December 14, 2021, workers performed truss installation on top of a wall of a residential construction rebuild, and was not protected by guardrail system, safety net system, or personal fall arrest system.
