29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the  requirement of this standard were being followed:   Plant 1, production floor, On or about November 30, 2021 and at times prior thereto, the employer exposed employees to potential hazardous energies in that the employer failed to conduct the required periodic inspection of the energy control procedures for machinery with multiple energy sources which includes but is not limited to the #5 Extruder machine. Employees were required to work on these machines with procedures that had not been audited periodically as required by the standard so that any deviations or inadequacies would be discovered and corrected.

29 CFR  1910.147(c)(7)(i)(A):Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:  a) Plant 1, production floor; On or about November 30, 2021 and at times prior thereto, the employer had not ensured that authorized employees were trained in LO/TO for Extruder machines, which contained electrical and hydraulic energy sources, exposing employees to electrical shock, amputation, caught-in, and struck by hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Extruder Machine #5 (Plant 1); On or about November 30, 2021 and at times prior thereto, the employer did not ensure that a rotating part was properly guarded in that the guard contained an approximately 3 inches wide opening, exposing employees to amputations and  caught-in between hazards.
