29 CFR 1910.178(n)(6): Industrial truck drivers were not required to look in the direction of, and keep a clear view of the path of travel: (a) On or about 11/18/2021 and at times prior to - 365 11th Ave W, Guin, AL., at the Holtec Saw Loading Platform, employees adjusting the cribbage (dunnage) to accommodate the placement of banded stacks of lumber were exposed to struck-by hazards because forklift operators delivering and placing the stacks of lumber were unable to see the operator when such adjustments were being made.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about 11/18/2021 and at times prior to - 365 11th Ave W, Guin, AL, at the Holtec Saw Loading Platform, employees adjusting the cribbage (dunnage) to accommodate the placement of banded stacks of lumber were exposed to caught-in hazards because employees were able to access the cribbage at the same time the stacks were being loaded onto the loading platform.
