29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: The employer is failing to protect employees from fall hazards while using extension ladders in residential construction activities. This was most recently observed on December 1, 2021 at 9552 South 183rd Ave, Omaha, Nebraska, when employees used an extension ladder to access the second story roof of a residential building and the ladder's side rails did not extend at least three feet past the roof eave.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: The employer is failing to protect employees from fall hazards while working from elevated surfaces during residential construction activities. This was most recently observed on December 1, 2021 at 9552 South 183rd Ave, Omaha, Nebraska where employees were working on the second story of a residential structure with no fall protection in place. The eave height was 12 feet above the ground. BradCo Companies, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1525105, citation number 1, item number 1 and was affirmed as a final order on July 9, 2021, with respect to a workplace located at 4507 186th Street in Elkhorn, NE.
