29 CFR 1910.178(l)(4)(ii)(C):The employer did not ensure that operators found to be operating in an unsafe manner were re-trained as per the requirements of this standard: a) Receiving Department/Inbound loading dock between door 184 &185 - On or about November 27, 2021, the employer exposed employees to struck-by hazards in that employer failed to ensure that employees were retrained when the evaluation revealed that employees were operating the powered industrial truck in an unsafe manner while employees were operating Crown standup lift truck # 33 (Serial # IA524933 & Model # RC5500) for unloading the freight.
