Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards:  a) Kanta Products Inc, Outside Steel Storage Racks North of Block Grinder Room: On or about November 29, 2021 and at times prior, employees were exposed to struck by hazards while conducting tasks such as, but not limited to: loading and unloading color and block machine molds from the storage racks that were not securely fastened to the concrete pad.  b) Kanta Products Inc, Outside Steel Storage Racks North of Block Grinder Room: On or about November 29, 2021 and at times prior, employees were exposed to struck by hazards while conducting tasks such as, but not limited to: loading and unloading color and block machine molds from the storage racks that did not display a plaque designating load sizes.  The employer failed to identify the load rating of the storage racks on the North side of the Block Grinder Room to ensure they were not overloaded.  c) Kanta Products Inc, Outside Steel Storage Racks North of Block Grinder Room:  On or about November 29, 2021 and at times prior, employees were exposed to struck by hazards while conducting tasks such as, but not limited to: loading and unloading color and block machine molds from the steel storage racks that contained damage.  The employer failed to identify and remove the damaged steel storage racking components.  d) Kanta Products Inc, Lifting Beam Lifting Device:  On or about November 29, 2021 and at times prior, employees were exposed to struck by hazards while utilizing an unrated septic tank lifting beam below the hook of the HIAB knuckle boom crane to move septic tanks.

29 CFR 1910.28(b)(11)(ii): The employer did not ensure that each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as described in 29 CFR 1910.28 Table D-2-Stairway Handrail Requirements:  a) Kanta Products Inc, Block Machine Stairs:  On or about November 29, 2021 and at times prior, employees were exposed to trip and fall hazards while accessing and egressing the flight of stairs consisting of 4 risers and 3 treads near the control panel of the block machine to access the concrete mold area.  The employer failed to ensure stairs contained a complete stair rail system on each open side.  b) Kanta Products Inc, Screener Infeed Belt Platform Stairs:  On or about November 29, 2021 and at times prior, employees were exposed to trip and fall hazards while accessing and egressing the flight of stairs consisting of 5 risers and 4 treads to perform maintenance on the screener hammermill.  The employer failed to ensure stairs contained a complete stair rail system on each open side.  c) Kanta Products Inc, Southeast raised parts room:  On or about November 29, 2021 and at times prior, employees were exposed to trip and fall hazards while accessing and egressing the flight of stairs consisting of 13 risers and 12 treads to gather parts from the parts room.  The employer failed to ensure stairs contained a complete stair rail system on each open and closed side.

The employer failed to develop, document, and utilize procedures to effectively control hazardous energy for employees that perform cleaning and mold change outs.    b) Kanta Products Inc, Mixer:  On or about November 29, 2021 and at times prior, employees were exposed to unexpected hazardous energy from sources of energy such as, but not limited to: electric, pneumatic, and hydraulic components while performing clean out of the Mixer.  The employer failed to develop, documents, and utilize procedures to effectively control hazardous energy for employees that perform cleaning of the mixer.    c) Kanta Products Inc, Screener: On or about November 29, 2021 and at times prior, employees were exposed to unexpected hazardous energy from sources of energy such as, but not limited to: electric, pneumatic, and hydraulic components while performing belt replacement, screen replacement, and greasing on the screener.  The employer failed to develop, document, and utilize procedures to effectively control hazardous energy for employees that perform maintenance and part replacement on the screener.  d) Kanta Products Inc, Splitter: On or about November 29, 2021 and at times prior, employees were exposed to unexpected hazardous energy from sources of energy such as, but not limited to: electric, pneumatic, and hydraulic components while performing blade changes and maintenance on the splitter.  The employer failed to develop, document, and utilize procedures to effectively control hazardous energy for employees that perform blade changes on the splitter.
