29 CFR 1910.147(c)(6)(i):  The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:   (a)	Izzio Artisan Bakery, LLC at 185 S. 104th St., Louisville, CO 80027:  On and before December 6, 2021, the employer did not conduct a periodic inspection at least annually of the Lockout/Tagout (LOTO) procedures used by employees in areas such as, but not limited to, maintenance, sanitation, and production. This condition exposed employees to thermal, amputation and other hazards associated with equipment such as, but not limited to, ovens, conveyors, and robotic machinery.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operators and other employees from hazards created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) Izzio Artisan Bakery, LLC at 185 S. 104th St., Louisville, CO 80027:  On and before December 6, 2021, the employer did not ensure that conveyor belts were guarded to protect operators and other employees from hazards created by the exposed underside of the belts on the conveyor system used to bring product out of the bread cooling unit. This condition exposed the employees in the racking area to injuries caused by unprotected rotating parts and ingoing nip points.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment had been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2):  (a)	Izzio Artisan Bakery, LLC at 185 S. 104th St., Louisville, CO 80027:  On and before December 6, 2021, the employer did not ensure through written verification that a workplace personal protective equipment (PPE) hazard assessment had been performed.  Abatement Note: The employer shall verify that the required workplace assessment has been performed through a written certification that shall include the following:      1)   Identifies the workplace evaluated;      2)   The person certifying that the evaluation has been performed;      3)   The date(s) that the hazard assessment was performed; and      4)   Which identifies the document as a certification of hazard assessment.

(a)	Izzio Artisan Bakery, LLC at 185 S. 104th St., Louisville, CO 80027:  On and before December 6, 2021, the employer did not ensure all operators were trained in the operation of powered industrial trucks and that the operator's training consist of a combination of formal instruction, practical training, and evaluation of the operator's performance in the workplace. This condition exposed employees to struck-by hazards when employees operated electric powered pallet jacks without having been adequately trained and evaluated.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and cables were used as a substitute for the fixed wiring of a structure:  (a)	Izzio Artisan Bakery, LLC at 185 S. 104th St., Louisville, CO 80027:  On and before December 6, 2021, the employer did not ensure that flexible cords (extension cords) were not being used as a substitute for fixed wiring in that, extension cords were connected to permanent equipment in at least one location of the bread racking conveyor system. This condition exposed the employees to potential electrical hazards.
