29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operators and other employees from hazards created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On and before February 21, 2022, the employer did not ensure that conveyor belts were guarded to protect operators and other employees from hazards created by the exposed underslung belts on the Rheon V4 4C180 Forming Conveyor system. This condition exposed the employees to injuries caused by unprotected rotating parts and ingoing nip points.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. (a) The employer did not report a work-related in-patient hospitalization incident to OSHA within 24 hours. On February 21, 2022 at approximately 1:30 AM an employee was injured when they became entangled in the belt and roller area of a conveyor system in the production area. The employee was transported to a hospital and admitted shortly thereafter. The employer reported the incident to OSHA on 6/23/22 at 4:37 PM MST.
