On or about November 29, 2021, the employer did not ensure employees were in use of eye protection when operating power actuated nail guns. This exposed the employees to a struck-by hazard.

On or about December 21, 2021, the employer did not ensure employees, performing roof work activities, were in use of fall protection. This exposed the employees to a fall hazard greater than six (6) feet to the ground below.
