29 CFR  1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  (a) JLG 400S aerial lift, serial #0300088285 - On or about December 7, 2021, employees operating the JLG 400S aerial lift were not protected from an ejection hazard where personal fall restraint equipment was not worn and connected to the basket of the lift.  (b) JLG 600S aerial lift, serial #0300130736 - On or about December 7, 2021, employees operating the JLG 600S aerial lift were not protected from an ejection hazard where personal fall restraint equipment was not worn and connected to the basket of the lift.

29 CFR  1926.454(a)(1):The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area.  Jobsite - On or about December 7, 2021, each employee onsite using the JLG 400s and JLG 600s aerial lifts was not trained in the hazards of working from an aerial lift without fall protection.

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  Jobsite - On or about December 7, 2021, each employee working on framing a dormer on a 5/12 pitch roof was not protected from a fall of approximately 18 feet where fall protection was not utilized.
