29 CFR  1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift.  (a)Jobsite - On or about 11-30-2023, employees operating the JLG aerial lift #N66195 were not protected from an ejection hazard where personal fall restraint equipment was not worn and connected to the basket of the lift.  Christopher Jackson dba CW Builders was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1567624, citation number 1, item number 1 and was affirmed as a final order on 03-29-2022, with respect to a workplace located at 452 Amherst Street, Nashua, New Hampshire 03064.

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe-boards, safety net systems, or personal fall arrest systems.                  (a) Jobsite - On or about 11-30-2023, employees were exposed to fall hazards while installing roofing materials at a height of approximately 16 feet above the lower level without any means of fall protection.   Christopher Jackson dba CW Builders was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1567624, citation number 1, item number 3 and was affirmed as a final order on 03-29-2022, with respect to a workplace located at 452 Amherst Street Nashua, New Hampshire 03064.
