29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) 206 Adeline Way, Collegeville, PA  19426, Roof:  On or about December 2, 2021, employee(s) were exposed to fall hazards of 30 feet to the ground.  The employer did not establish a program that adequately trained employees to recognize fall hazards.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) House Exterior Wall, Right Side:  On or about December 2, 2021, employees used a portable, 24-foot aluminum extension ladder against the house exterior wall (right side) to access a roof, but the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface.  (b) House Exterior Wall, Rear :  On or about December 2, 2021, employees used a portable, 24-foot aluminum extension ladder  against the house exterior rear wall to access the roof, but the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface.

29 CFR  1926.1053(b)(16):Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately.  (a) House Right Side, Exterior Wall:  On or about December 2, 2021, photos taken by the Upper Providence Police Department upon their arrival to the accident scene on the day of the accident showed that employees used a 24-foot Louisville extension ladder that was damaged (bent rung), to access the roof of a two-level residential structure.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    (a) 206 Adeline Way, Collegeville, PA  19426., Roof:  On or about December 2, 2021, employee(s) performing roof work from a multi-story pitched roof were not protected from fall hazards of 30 feet to the ground.  Maximum Contractor Corp was cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1530118, citation number 1, item number 1, and was affirmed as final order on September 10, 2021, with respect to a workplace located at 2&4 Maplewood , Lakewood NJ, 08710.

29 CFR  1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident    (a) 206 Adeline Way, Collegeville, PA  19426.: On or about December 2, 2021, an employee suffered fatal injuries after falling 30 feet off a roof, but the employer did not report the employee's fatality to OSHA within 8-hours.
