Establishment: Employee number 3 (Production) sustained a Standard Threshold Shift  of 18.3 dB with a hearing level of 63 dB  when averaged at 2000, 3000, and 4000 Hz range in the left ear and had not been recorded on the year 2021 OSHA 300 and 300A.

29 CFR  1910.95(g)(8)(i): When a comparison of the annual audiogram to the baseline audiogram indicates a standard threshold shift, as defined in 29 CFR 1910.95(g)(10) has occurred, the employee was not informed of this fact in writing, within 21 days of the determination:   (a) Facility wide:  On or about August 12, 2021, when a comparison of the annual audiogram to the baseline audiogram  on July 12, 2021 indicated a Standard Threshold Shift, as defined in 29 CFR 1910.95(g)(10) had occurred the employer did not inform the employees of this fact in writing, within 21 days of the determination for the following 3 employees:  1.Employee number 1 (Saw Room) suffered an STS  11.7 dB with a hearing level of  36 dB when averaged at 2000, 3000, and 4000 Hz range in the right ear.  2. Employee number 2 (Production) suffered an STS of 16.7 dB with a hearing level of 25 dB when averaged at 2000, 3000, and 4000 Hz range  in the right ear.   3. Employee number 3 (Production) suffered an STS of 18.3 dB with a hearing level of 63 dB  when averaged at 2000, 3000, and 4000 Hz range in the left ear.
