A relocatable power tap used to power the Dayton pedestal fan was energized through an extension cord, which is not an approved use for a relocatable power tap as stated in the UL Whitebook. B. A duplex handy box designed for surface mounting was free hanging from the inner conductors provided in the armored cable that was energizing the box.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: Paint Area: On, or about, September 13, 2024, the following electrical equipment had not been provided with adequate strain relief: A. The power cord that energized the Dayton pedestal fan Model 4C458F was pulled from the fan casing fitting and wrapped with electrical tape. B. A duplex handy box that energized the both the Dayton fan and lighting in the large spray booth was hanging from the inner conductors that energized the box. Armored cable had detached from the box fitting, leaving the electrical conductors to suspend the box.
