29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: (a) Shipping/Receiving: On or about December 1, 2021 and times prior, employees unloaded lumber from railcars using a man lift attached to the forks of a fork truck approximately 10.5 feet high. No prior written approval for the use of the man lift could be produced.
