29 CFR  1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a)  21st Century Production floor- ARI Lab area: Employees were exposed to hazardous chemicals such as, but not limited to magnesium oxide,  magnesium fluoride, acetic acid, silicon dioxide, and sodium hydroxide; while operating and cleaning a computer control chamber applying an anti-reflective coating to optical lenses.   b)  21st Century Production floor- Blocking Area: Employees were exposed to hazardous chemicals such as, but not limited to an alloy mixture containing bismuth, lead, indium, tin, and cadmium; while operating a blocking machine attaching a metal block to the optical lens.  Hazard communication training was not in place; on or about 11/26/2021.  Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

29 CFR 1910.134(c)(2)(i): The employer did not provide respirator users with information contained in Appendix D to this section("Information for employees using respirators when not required under this standard") when voluntary respirator use was permissible:  a)  21st Century Production floor- ARI Lab area: Employees voluntarily used a 3M 5101 half face elastomeric respirator while cleaning and loading hazardous chemicals, such as but not limited to magnesium oxide, magnesium fluoride, acetic acid, silicon dioxide, and sodium hydroxide into a computer control chamber applying an anti-reflective coating to optical lenses.   b) 21st Century Production floor- Sandblasting Area: Employees voluntarily used an MSA 808076 Comfo Classic elastomeric respirator with particular filters while engaging in an enclosed system sandblasting operation.  During this process, employees removed excess anti-reflective coating materials from the lens.   Information contained in Appendix D was not provided by the employer; on or about 12/15/2021.  Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
