Material containers stored in tiers were not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:  (a) On or about April 13, 2022, the employer did not insure case boxes of contact lenses stacked on top shelves of storage racks were stacked, secured and positioned  in a manner to prevent the accidental displacement of boxes. This exposed employees working at and around the racks and in the adjacent aisles below to a struck by hazard.
